Proof of COVID-19 Vaccination Receipt

While the Province develops a centralized vaccine passport to be available Oct. 22, those wishing to enter movie theatres, gyms, restaurants and other non-essential indoor spaces in Ontario will require proof of vaccination along with proof of identity as of September 22.


During the month between those dates, or to provide evidence of full vaccination as already required by many employers, businesses and event organizers, you can download your electronic COVID-19 vaccine receipts by logging onto the Province's COVID-19 portal. There you will find receipts for both your first and second shots taken anywhere in Ontario. You can save the electronic receipt on your phone, or print it out and keep it on you.


To log into the portal, you will need your green photo ID OHIP card, your date of birth and your postal code.


If you have a red and white health card or do not have one at all, you can call 1-833-943-3900 for help in getting your receipt. If you received your first or second shot outside of Ontario, you will have to contact the local public health where you received your shot to request your vaccination information.
